Errors from (unspecified) BASIC program?

The Question is:

 
Getting logged out of application with this error:
%bas-e-fatsysio_,fatal system i/o failure
then
-bas-i-on-chfil, on channel 1 for file dsa2:
[c60.user](our file names) at user PC 00000000
-rms-f-ipi, invlaid internal file identifier
(IFO) value.  Then the message repeats itself
for all the files that were being accessed at
the time the user was logged back to a system
prompt. This is an intermittent problem, not
occurring at any specific time within the
application.  At one point the user was typing
and got logged out while typing.

The Answer is :

 
  This resolution of this problem will require the assistance of the
  particular organization(s) supporting the particular application.
 
  If you are the organization supporting the application, you will
  need to determine the particular footprint of the error -- this
  could be an application bug, a data corruption or stack corruption
  problem, some sort of problem in the particular version of BASIC in
  use, or a problem in the BASIC run-time library.
