The Answer is :

 
  The application or one of the underlying packages appears to contain
  a bug -- specifically, something is apparently triggering an access
  violation.  Check for ECOs for ACMS, DECforms, and OpenVMS, as well as
  contacting the support organization for the vendor of the application
