MEDOFL Medium Offline Errors?

The Question is:

 
Disk won't MOUNT
MicroVAX II running VMS 5.1.  RD53 hard drives.
 
Show device dua1 shows that dua1 is online.  However, when I try to mount
dua1 it reports the medium is offline.  What can I do to correct this
problem?

  This error generally indicates a hardware or configuration problem
  with the controller, with the Q-bus, or the specific disk.
