Cause of hardware errors?

The Question is:

 
We have Two ES40s and 1 DS20s in cluster with RA8000 storage.  We are facing
 excessive errors in the SCSI disks in openvms, connected to HSZ80 controllers
 in both multibus and transparent failover modes.  What may be the reason for
 this.  The recent SCSI
patches were also applied.
