Cannot MOUNT or INITIALIZE disk?

The Question is:

 
I have installed an rz1df-wa scsi disk as dka100
The disk shows as online, however it will not mount or initalize.  When
 mounting, an error message is generated asking for the device to be inserted.
 
Any ideas?

The Answer is :

 
  The disk device is misconfigured, miswired, or malfunctioning.
  Please contact your hardware service organization for assistance,
