(1) bootable recovery tape/floppy and (2) formatting disks

Having acquired a decstation 200 4/166. I have a couple of questions:

(1) HOw does one make a recovery/boot tape on a DAT or a floppy?...I am
looking for the equivalent of a recovery tape on an HP/9000-7xx(hpux) system
or of a "standalone backup" tape/floppy on an old microVAX/VMS system.

(2) What is the command to initialize/format (low-level) a hard disk?
I am not looking to put a new file system on or to label the disk. I am
looking for the equivalent of the "mediainit" command in HPUX, of "rzdisk"
in ultrix or of INIT/ERASE under VMS.
