Recovery from bad disk part of Log Volume

I have recently taken over the administration of a DEC ALPHA running
OSF/1 Ver 1.3 and confronted my first problem when one of the four disks
defined as part of a logical volume had to be replaced.

After physically replacing the disk, what is the proper series of steps
to get the new disk into the volume group?

I worked around my lack of knowledge by renaming the /etc/lvmtab and
starting over as if I never had a volume group or a logical volume.

My system admin manual is clear on setting up the virtual disk but does
not go into what happens when one or more of the physical disks where
the physical volumes reside must be replaced.

Please respond by email. I hope my next brush with this problem can be
more efficient than the first. Thanks.
