Megasafe internal inconsistency error at boot time

Good day,

    We recently had some hardware problems on our 2000/400 running OSF
V3.0B (the raid controller and one disk of a raid5 set crapped out).
After replacing the hardware, I tried to boot the system and got the
following message (although it may not be exact, it should be very close):

Mega Safe internal inconsistency encountered

This message came up right after it tried to mount the root advfs file
system. After the error, the system returned to the console prompt. I
do know that instead of forcing a rebuild of the fialed drive after
replacing the hardware, the DEC dude made the failed drive optimal (even
against the dire warning that is displayed about making a fialed drive
optimal can have catastrophic effects).

What does this message mean (apart from reinstall OSF), and what would
cause it?
