System crash - please help!!

Hi all,

        we are running DEC 4000/620 and DEC Unix 3.2A, FW revision 3.5
(the system was upgraded from OSF 3.0 & FW 3.4). This machine should serve
as file and DB server => this machine *SHOULD* be stable. Unfortunately
from time to time (1x per 1-2 weeks) server crashes with the following
error message:

In addition the following error are reported (by uerf) every time machine
boot (independently on the SW/FW versions).

But no one (including our DEC representative) can explain this message :-((

No other SW or HW errors or warnings are reported :-((. Our local DEC
representative are unable to get any help or suggestion.

What is wrong ?? Any help or suggestion ??
