The problem was that the root filesystem needed to be fsck'ed. After
the machine was incorrectly shutdown, fsck did not run on /. Booting
into single-user mode and fsck'ing manually fixed the problem.

Jan Mark Holzer pointed out that /etc/fstab is incorrect on systems
shipped before October 18. This is why fsck didn't run properly on /.
Here's what he says:

Incorrect fstab entry fis'd prior to 18-Oct-95:

                /dev/rz0a / ufs rw 1 0
                /dev/rz0g /usr ufs rw 1 1

        Correct fstab entry:

                /dev/rz0a / ufs rw 1 1
                /dev/rz0g /usr ufs rw 1 2

Thanks for the help, folks!

Matt Harrington
