Problem adding a disk in LSM

We received an AlphaStation 400/200 and configured it like our 3000
series computers with LSM and ADVFS.
On my other TurboChanel machine when I want to add a new disk into LSM I
simply plug the device in the StorageWorks box, give it a label and init
it into LSM ... I.E.:
# disklabel -rw /dev/rrz13c rz28m
# voldisk init rz13c type=nopriv

This works fine on our Turbo chanel stations, but on the new 400/200
(PCI) I get the following error when doing the LSM initialisation:

Error checking for overlapping partitions: cannot determine swap device
file name


My disklabel looks fine:

Does anyone know how to get out of this one???
