boot errors halt system

One of our researchers has a new Alpha 200. The system haug and was
powered off and now hangs on reboot with the following:
dirty filesystem on /usr, then
   update started
   kmknod:getsysinfo() or mknod():
   File exists

....hangs

I can interrupt the boot, manually fsck /usr, mount it and finish booting
with init 3. The three error lines above still occur, but the boot finishes.

On clean shutdown and reboot though, the errors re-occur, /usr is still
dirty and the system still hangs.

Any clues?
