This is what I found regarding the missing 'extendfs' command:
nicolis_at_celfi.phys.univ-tours.fr states that he has got the command both in
3.0 and 3.2a manpages so I guess he must have done some 'installupdates'
considering the answers from DEC people:

manual page had been present since V1.2. I'm told that
the reason it was removed is that it would "compete"
with addvol from the Advanced File System.

Yes, at least the man page existed in the 3.0 release - I saw it, but never
checked to see if the command was there. In any case, it looks like it
was put on 3.0 by accident, and it was never a supported part of Digital
UNIX. Perhaps it was a vestige of the the OSF/1 {from OSF} reference code??
Looks like we removed it.
Anyway, I most certainly believe the command was present as late as in 3.0
(both in real life and in the manpages. Anyone willing to check ?) but it
seems like it never should have been there. This was not good news to me
because it would have been very nice to grow a filesystem on the fly without
having to buy the 'AdvFS Utilities' license which is quite expensive.
P.S I've used the 'growfs' command on SGI logical volumes several times. Very
neat and time saving !
