Sorry for such a late answer, I was fighting with the disk.
The current version of Advfs panics on I/O errors which corrupt
the file system or when they detect corruption. It is a bug
in Advfs, and digital offers a patch (but it's not a public patch).
Anyway, the best thing is to replace the disk.
