I solved the problem by loading the version of dxnotepad from the Digital
Unix 3.0 CD. It has worked fine ever since.

Jim Skoog says that there is a patch for this problem which replaces the Motif
shared library (libXm.so). I didn't investigate this one further because the
3.0 dxnotepad works.
