AdvFS-generated crash

Hi all

Have anyone experienced crashes while defragmenting or heavily accessing
an AdvFS filesystem ?. We have an Alpha 2100 4/200 with DU 3.0B and a
KZPSC RAID controller. On one channel we have the root, usr and swap
disks (RZ28-VA), while on the other channel we have a RAID 5-level array
of 5 disks.

Everything was working fine more than one year, till 6 weeks ago, when
suddenly, it crashed down, showing an "Advfs exception" in the
/var/adm/crash/crash-data file and hardware errors (but not bad blocks)
related to the disk 0 on the binary.errlog. After this first crash, we
decided to change the KZPSC, the disk 0 and the SCSI cables, and it
worked by 1 more week. From then on, the machine is crashing almost
every 3 days. We have noted that it crashes while accesing heavily the
disks.

One test we did was to defragment the root and usr domains, despite its
low fragmentation level (97% and 94% of aggregate IO performance
respectively). It worked fine. While defragmenting home_domain (on the
RAID 5 array) the machine crashed with the same characteristics.
