SCSI Device Problem

Hi,
Recently we had a Digital service engineer install a TLZ7L-DA autoloader and
an additional 4GB harddisk (rz29b) in our Digital AlphaServer 2000 4/233.
This server currently has 1 2GB harddisk (rz28), 4 4GB harddisk (rz29b), 1
internal cd-drom, 1 internal tape drive (TLZ07), 1 external tape drive
(TLZ07) and one external autoloader (TLZ7L-DA). The kernel configuration
file for the SCSI setup is as shown below :-
controller scsi0 at psiop0 slot 0
device disk rz0 at scsi0 drive 0
device disk rz1 at scsi0 drive 8
device disk rz2 at scsi0 drive 16
device disk rz3 at scsi0 drive 24
device disk rz6 at scsi0 drive 48
device tape tz5 at scsi0 drive 40

controller scsi1 at psiop1 slot 0
device tape tz14 at scsi1 drive 112
device tape tz13 at scsi1 drive 104
device disk rz8 at scsi1 drive 64

I noticed that in the /var/adm/messages file the following error message is
displayed during bootup :-
Oct 29 18:14:12 omc_pg4 vmunix: cam_logger: CAM_ERROR packet
Oct 29 18:14:12 omc_pg4 vmunix: cam_logger: bus 1 target 5 lun 0
Oct 29 18:14:12 omc_pg4 vmunix: ss_device_reset_done

And when I try to run Advfs utilities I get the following error message in
the console :-
Oct 29 18:16:13 omc_pg4 vmunix: fd failure: No media in drive.

Another related problem is after the installation was done a few hours later
the server suddenly froze for a few minutes and the /var/adm/messages file
recorded the following error messages :-
Oct 28 20:44:15 omc_pg4 vmunix: Reached max abort count, scheduled bus reset
Oct 28 20:44:15 omc_pg4 vmunix: cam_logger: CAM_ERROR packet

Oct 28 20:44:15 omc_pg4 vmunix: ctape_async
Oct 28 20:44:15 omc_pg4 vmunix: Bus reset notification
Oct 28 20:44:16 omc_pg4 vmunix: Hard Error Detected
Oct 28 20:44:16 omc_pg4 vmunix: DEC TLZ07

My question here is how do you identify what is the problem here and what is
the cause of the problem, and how do you fix this problem.
