Problem with KZTSA

We're attempting to add a KZTSA SCSI controller to an Alpha 3000/600; the
board installs, is detected, but so far we've been unable to access the
attached disk.

The drives show up from diagnostics; we're perhaps missing something very
basic (since the doc CD was missing from our unit, and getting it replaced
is taking quite a bit of time).
