The original question:

Could someone please send instructions for how to configure DU 4.0 on an
AlphaStation 233/255 to use an additional SCSI adapter?

The answer:

Reboot using /genvmunix before running doconfig to make a new kernel.

It's that simple, and that counterintuitive. Thanks to all--this
should be an RTFM, but sadly I only have the docs on CD, which doesn't
always help when one is monkeying around in single-user mode.

For the curious, or manually inclined, the kernel config lines it added:

bus psiop1 at pci0 slot 12 vector psiopintr
controller scsi1 at psiop1 slot 0
