The PXG family of graphics cards are not supported in V4.x of Digital UNIX.
The release notes for V3.2C state:
"The DDX and other drivers to support PXG-type devices will be retired
in the next major functional release of Digital UNIX"
Your options are to stay at V3.x or to upgrade to a ZLX graphics card.

If I remember correctly PMAG-DA is a PXG card. These are not supported
and don't work with DU 4.0

This may be one of the cards that DEC seized to support from release 3.2C
and upwards (contact DEC or look in the Release Notes) or it may be you
have to install OPEN3D.

That graphics card is no longer supported by Digital UNIX or Open3D. You
have two choices: (1) install an older version of Digital UNIX in which
the card is supported, and also install Digital Open3D if you need to use
the card for 3D graphics applications, or (2) get a newer, supported card
for your workstation.
Of the two, the latter probably makes more sense.

I am sorry, but support for the PXG graphics line (which includes
the PMAG-DA card) was retired as of Digital UNIX 3.2. No support
for this card is available under UNIX 4.0, and the GENERIC driver
(designed for EISA and PCI cards) wont work with this card. If
you wish to run under Digital UNIX 4.0 you must replace the
graphics card with a supported model such as a ZLX-E1 card (PMAGD-AA).
