My thanks to the following:
Mark Zander: <mark_zander_at_spcc.com>
Brian O'Connor: <boc_at_ironbark.bendigo.latrobe.edu.au>
Robert Schuhl: <robert.schuhl_at_ald-vt.de>
Tom Webster: <webster_at_ssdpdc.mdc.com>
Bugs Brouillard <bb1_at_axe.humboldt.edu>

Bugs was right on with:

The firmware image file was marked readonly, and thus, on a PC:

DOS> attrib -r swxcrfwa.216

did the trick. I'd guess that the author of the loader
(swxcrfw.exe) forgot to open the file for readonly.
