I ran into this problem today and searched the archive.
I saw a few messages, but no easy solution.
I went to the /etc/auth/system and edited (vi'd)
the ttys file.
This is the line in question:
local\:0|local\:0.0:t_devname=local\:0:t_xdisplay:t_login_timeout#0:chkent:
\:0|\:0.0:t_devname=\:0:t_uid=root:t_logtime#859991088:\
:t_unsucuid=root:t_unsuctime#860074439:t_failures#12:t_xdisplay:\
:t_login_timeout#0:chkent:
All I did to fix the problem was delete "t_failures#12:"
At the login prompt I hit the cancel so it would refresh and I logged in
with no problem at all.
