ATMworks 350 PCI in Alpha 1000A

I just received an ATMworks 350 PCI ATM adaptor for an Alpha 1000A
5/333, and am having problems getting it installed and configured.

I've read the summaries about this card, but nothing quite seems to
match. The error I get upon bootup (I'm booting genvmunix, so's I can
make a new kernel) is the dreaded 'Module 111a:2 not in pci option
table, can't configure it' error message.

I just upgraded the firmware to the V3.9 CD, putting me at v4.8.

Somewhere I saw mention that this card needed firmware v5.0 (assumedly
because the options table includes this particular card).

Um. Well, if the latest firmware on their FTP site is the V3.9 CD and
that's gives v4.8 firmware, how does one get their firmware up to v5.0?

(Or, more to the point, how can I get this card working under DU 4.0B?)
