Thanks to everyone who answered. The solution was really quite simple in
the end.
As per Digital SA:
>>>Ensure that you set VGA ENabled jumper on the 3D30 module which
is inserted into Slot 11 of the PCI. (refer to the Owner's or
Installation Manual)
>>>Ensure that the VGA enable jumper is DISabled for 2 of the 3
>>>3D30 modules which should be installed into Slots 12 and 13 of the PCI
bus.
Our hardware engineers did not have the primary video card in slot 11.
Once this was done, everything worked fine. (We did search in the
documentation but did not find this critical piece of information.)
Also note that 2 of the 3 screens come up blank - only the primary screen
comes up with the CDE.. The mouse pointer however does traverse all 3
screens. Use something like xterm or dtmail to test the display on the
other two screens with the "-disp :0.1" option.
As you can see these alphastations do support 3 screens and is running
the open3d software.

Hello osf-managers,
We have Alphastations 255, 233Mhz and 300Mhz configurations with Factory
installed software Unix 4.0b. The system installation goes through
successfully but as the system initialises the 3 graphics controllers and
attempts to start the CDE environment the system crashes and starts
dumping.
These Alphastations come with 3 X Digital TGA2 graphics controllers (part
# PBXGB-AA/CA PCI graphics cards).
Firmware levels:
SRM: 6.4-2
OSF-Palcode X1.46-2
Serial Rom ver 1.12
Diag Rom ver 1.5
While the system is booting, the 3 graphics cards are recognised by the
kernel. We tried 3 different approaches: 1. With one card the system
boots fine. 2. With 2 cards the system also boots fine but only one of
the screens display the CDE the other one stays blank. 3. With 3 Cards
the system panics with a kernel error message.
Do you know of any problems regarding this configuration?
Is it even supported with 3 graphics cards?
Are there any patches that should be installed for this problem?
Perhaps there is a file which needs to be edited in single user mode for
it to support more screens???
