display crash on 500/333

I have a 500/333 running DU 3.2 62 alpha that appears to be
experiencing crashes of the video display. X dies and this error
appears:

   Jun 6 16:05:32 eiger vmunix: Defering I/O (errno 5) for block(0x#####,
   0x#####) on device 8,6

Initially it said (errno -1) and then switched to errno 5. The block
address keeps changing.

This is occasional, and the machine is remote (in another city), so
I've had trouble observing the problem myself.

It's running OSF PALcode V1.21-0, and has a PBXGB-AA graphics card.
Has anyone else seen this? Would a firmware upgrade be likely to fix
it, or is it some other kind of problem?
