We've had exactly the same problem (on an Alpha 2000) but the number of
errors occured much more frequently and randomly on different target
numbers. I logged the call a number of times with Digital SA mainly
because I was never satisfied with the response. They initially didn't
know why this was so. As I pressurised them into giving me a decent
response they eventually succumbed with the following:

Question 1
The disk errors are allways logged to the binary_error_log.
Please provide the /var/adm/binary.errlog & RCU Revision.
Check if the configuration of the RCU is correct (Fault
Management Enabled) and upgrade the Firmware version to
ensure that it is not a bug.
Question 2.
I remember giving advise to Paulo on this issue. If the errors
are BBRs and quite often ( almost daily ) replace it ASAP. If
the errors are ( 1 or 2 monthly ) monitor the System.
Enable swxcrmon -m.

These errors happened at least once or twice a month. The SWXCR gui
interface(Motif) is very pretty but seems to be buggy - this tool also
didn't help very much and never once reported soft or hard errors even
though they were being reported via the SWXCR daemon mailing to root.
