C2 problems

Hello,
 today I found following errors in our database. I must say, I didn't make
any changes during some time. One user told me - in the morning everything
worked, now it doesn't. They couldn't simply log-in. "Login incorrect".
I added them manually by 'convuser -a -v -i username', but have some
question about u_oldcrypt option, see below. BUT one problem still
remains. I din't found anything related to it in syslog.

Questons:
1. How to get away of the " " entry?

2. What encryption methods are those u_oldcrypt#0, u_oldcrypt#2, and I
hope there are some others? Which one is best?
