Installation of Digital Unix 4.0B on AS1000A

I am trying to install Digital Unix 4.0B on AlphaServer 1000A. After I
boot from the CDROM using >>>boot dka400, the boot process terminates
abnormally with the message "Unable to create FRU table" and "Memory
fault".

Will greatly appreciate if anyone can provide a clue as to what is
happening here.
