[Q] tlz06 errors

Hi DU Admins

Since last Friday evening we've been getting a lot of errors
from our tlz06 tape drive.

Most of these errors are for routine name ctape_iodone, some
for ctape_wfm and a few for ctape_move_tape. I've included a
representative uerf output from a ctape_iodone error below.

Can anyone tell from this if this is a hardware problem or
could it just be a bunch of bad tapes? I recently ran a head
cleaning tape through the drive so it shouldn't be a result of
dirty heads.
