For the ddr.db problem all replies suggested rebuilding the kernel
and/or re-creating the file with ddr_config. None of these
suggestions worked. But today I did 2 things,

1) Upgraded the firmare in my machine to V7.0

2) Changed the (uid,gid) of ddr.db to (3,4)==(bin,bin). I noticed
that the owner and group was inconsistent with the numbers listed in
/usr/.smdb./OSFHWBASE400.inv, though I have no idea how it happened.

One of these changes, most probably the second, cured the problem.

My lmf database had become screwed up so I issue'd all the duplicate
PAks to sepereate files, and reloaded one of each. Now my lmf
database seems more organised!

I ran "uerf -R -r 300" on my 3000/300L and I came accross this from
recent boots
 DDR: WARNING: Unable to obtain Device 
  _Information Database 
      Unable to open file /etc/ddr.db 
 DDR: WARNING: Basic Defaults will be 
  _used. 
This message has been sent all of the 21 times I (or someone else)
booted the machine since my upgrade to DUnix V4.0 (21 seems a lot!).
Since the system seems to be functioning correctly it's currently not
a problem, but I can see the file /etc/ddr.db (and the text version
/etc/ddr.dbase), so why can't the system.  It's on the root partition
of my boot (SCSI) device, and the permissions on these files are 755??
Also my lmf database seems to have got mixed up, or has it?  If I do a 
# lmf list | sort

I must admit to not understanding lmf at all really, and find it a
huge pain whenever I have to install new software/versions (which
isn't my job, but if I _need_ the software then I have to do it myself
....)
