Thanks to several folks who answered, I found the correct patch for
this in duv32gas00004-19980219.tar.

Original question:

We have an Alphaserver 2100 with 2 5/250 CPUs, running 3.2G. Twice
this week (Sunday night and now Thursday night) it has crashed in the
   same way:

      simple_lock: time limit exceeded

      pc of caller: 0xfffffc000042d368
      lock address: 0xfffffc0007338238 (Sunday)
      lock address: 0xfffffc000725de38 (Thursday)
      current lock state: 0x0000000000282e59
                                   (cpu=0, pc=0xfffffc0000282e58, busy)

      panic ( cpu 0 ): simple_lock: time limit exceeded
      syncing disks...

   and it hangs there.

Since this has happened twice in fairly close succession, I am
   concerned. What could this be?

Thanks, everyone.
