useradd error in 4.0D

Hi,

after an upgrade from 3.2G to 4.0D, useradd is returning:

useradd: Two entries with the same group name but different GID's were
found

and exits. Nothing strange is present in /etc/passwd nor /etc/group. A
while back the same problem was posted but I was unable to find a
summary with the solution. Does anybody recognize the problem?
