I have a Alpha server 4100 with a passwd file with approximately 3700
entries.
Running DU 4.0B standard security.
Starting Friday when one of my staff was adding users with addusers, he
received the following:

Adding new user...
Rebuilding the password database...
mkpasswd: passwd: File exists
Do you wish to edit the auth file entry for this user (y/[n])?
Creating home directory...
You must enter a new password for (jritt).
Changing password for jritt.
New password:
Retype new password:
Hashed passwd database does not match the /etc/passwd file. Rerun 'mkpasswd
/etc/passwd'.
Warning, the password for (jritt) was not set.
Finished adding user account for (jritt).

He then reran mkpasswd /etc/passwd and recieved
mkpasswd /etc/passwd
mkpasswd: /etc/passwd: File exists

My suggested workaround is to use vipw to force an mkpasswd and get the
hashed database up to date.
Other things tried:
Deleted passwd.pag and passwd.dir files and recreated the hashes with vipw.
