sendmail 8.9.1 relaying/hosts allowed?

I know that sendmail 8.9.1 by default prevents relaying, but we have a
server that is accessed all over, and I can't possibly get all allowed
hostnames...

1: how do other's handle this for people within the subnet using imap and
pop (I assume you can allow "*.domain.edu" to help with this), all over the
world, but still deny spam relaying?

2: how can I set it up like geocities, where it won't allow you to send
mail until you authenticate with pop first, then it gives you X minutes to
send mail before having to reauthenticate?

3: what is the format of the sendmail.cw file? My O'Reilly book is the
older ed. and I have nothing to help me with this.
