dump error

Hellow every one ,

 
 I have facing some problem while i am taking backup . When i issue the
command " $dump -0uf /dev/nrmt0h /usr"
 
then after few seconds it gives error and dumping aborted due to this:

error message are:

dump: write error -wanted write:10240 ,only wrote -1
slave_work():write(): I/O error
dump:write error:/dev/nrmt0h ,volume 1,6 feet --cannot recover
dump: NEEDS attention: Do you want to restart this volume ? : ( yes or
no)
dump:cannot remove shared memory
remove_shared_memory():shmct():invalid argument
dump:SIGTEM received

I am using digital TLZ07 DAT tape .
most of the time I have completed backup successfully but some time it
gives error like above . I have two same decalpha servers . when the
probelm occuerd in the first machine then for testing point of view i
tested out it in another machine and taking some backup it is successfully
done
now the sam tape if i am again put it in the first machine ( which is
concern ) and first move it by using command $ mt fs 2 and then taken
backup it is o.k but if first i move as $ mt rewind then it gives
error as above . I am taking live backup i.e in multiusermode. I have
tested thess by using two or three tapes, but the same problem occured.
Please try to solve the problem.

And further tell me how many times I can reuse a tape ( means
overwrite ) .
