useradd error: "Two entries with the same group name..."

I'm trying to run the adduser script for the first time since we
upgraded from 3.2 to 4.0d. After I return when it asks for login shell
it says:

Adding new user...
useradd: Two entries with the same group name but different GID's were
found
Error, exiting useradd /usr/sbin/adduser. Check /etc/passwd.

I couldn't find two entries with the same group name but different GID's
in my group file and couldn't find anything strange in the passwd file.
I then tried to just do a simple useradd command and received the same
results:

# useradd -D
useradd: Two entries with the same group name but different GID's were
found

Does anyone know what the problem might be? TIA
