As many people pointed out, the SCSI or RAID man page details much of
this info.
What I am looking for, (Contraily to what my previous message seemed to
say) is
not so much as a mapping, but a complete list of each type of device
available
from the console and Digital Unix.
Here is what I have so far:

If anyone has any of the info to fill in the blanks or additional types,
please respond and I'll send out the list a few more times to try to
gather as much info as possible.
