Alphaserver 1000A an Disks problems (no such device)

We have an Alphaserver 1000A with 4 PCI SCSI-Controllers : Normal SCSI
                                  1 PCI SCSI-Controller : Wide SCSI


When powering off/on the Server mounts all disks: >>>sho dev all disks seen
                                        single user boot all disks seen
                                        multiuser boot all disks will
                                                                be mounted.

But:
After crash or shutdown/init 0 some strange things happen:

>>>show dev all disks seen
single user boot all disks seen
mulituser boot some disks are not mounted, error message: no such device !!!

The disks with the error message are not always the same. But the disks which
are not seen are all on the same PCI-SCSI-Controller at this time. Next boot,
other PCI-SCSI-Controller is not working correct (disks are mounted: no such
                                                device!!!)

in console mode we set: set boot_reset on

The motherboard/cpu is replaced by a new one, same problems.

Any suggestions?
