Can't kill Loader Process

On an Alpha 8400/Unix 4.0d we've had a couple of Loader executions get
in some kind of race state that we are unable to kill. The file that
the Loader is accessing is NFS served and the Loader executable is
coming from a local ADVfs file system.

Any ideas on this?
