AS1200 crash with Digital Unix 4.0D

Dear alpha-osf-manager:

QUESTION:

        I have an AS1200 with DigitalUnix 4.0D. Some times the server monitor
(with CDE and the mouse) is locked and completely black. After tha the
server go down and never user can work with it. I don't now what happen? Is
a hardware problem? Is a software problem? I don't have installed the last
paches for DigitalUnix 4.0D, it is necessary.
