8400 sometimes loses CDROM drive

A strange, intermittent problem this one. We have two 8400's at firmware level
      4.9

Sometimes, following power up (disable -> enable) the CDROM (RRD45) isn't seen
      during the SCSI probe. No error messages are reported, but it just doesn't
      appear in the show dev output. Pressing the eject button doesn't open the
      drive, so it looks as if it hasn't been seen at init time. The same
      problem appears on both boxes about 25% of the time. Switching off and on
      is the only way I know to fix it.

P.S. We're currently sticking with firmware 4.9 because 5.0 comes with a caution
      about problems with KZPSAs.
