The actual piece(s) of information I needed were:
rz<LUN>(8*<bus #> + <SCSI ID>)X for the MAKEDEV command
and
The form of the unit name on the HSZ side is:
D#0L
Where '#' is one if the IDs and L is the logical unit number you want the
host to see.
