Shared memory problems with ingres and 4.0d + patch kit 3

Hi,

I installed patch kit #3 on top of 4.0d at the weekend and now ingres
crashes after about 50 users login with the following error:

Now the Ingres guys say it's an operating system problem, and the fact
that i've applied the patch at the weekend seem to back this up. Has
anybody out there any ideas for places to look.

My kernel parameters are all set large:
