Well, after many attempts, nothing worked to get my second CPU "alive"
so I reinstalled the OS after making sure the 2nd CPU could be seen
at the boot prompt.
That seems to have done it as we now have;
# psrinfo -v
Status of processor 0 as of: 06/09/99 10:12:25
Processor has been on-line since 06/09/1999 09:01:12
The alpha EV6 (21264) processor operates at 500 MHz,
and has an alpha internal floating point processor.
Status of processor 1 as of: 06/09/99 10:12:25

as a further summary for the archives;
- you need a second OSF-BASE license for SMP systems
- try booting into genvmunix (boot -fi /genvmunix)... didn't help in my case
- top does not support SMP systems
