Determine type of SCSI-controller

Hello,

I wonder if anybody here might be able to give me some suggestions about
the part number of a SCSI controller of ours. We had a KZPAA-AA that
Digital replaced with this card. We are in the process of fixing a new
maintenance contract and we need that part number.

The card is an ultra wide, single ended, single channel device, and it is
identified by Digital Unix as:

isp1: QLOGIC ISP1040B/V2

I tried to get some use out of the former storage.digital.com, but I
failed. Any takers?
