Things to look for:

There's a bug in the license manager that shows up with installupdates.
Bug with X, xdm logins.
Must re-install license pak after a fresh OS install.
Use command 'lmf list full for OSF-USR' for more info.
100 user license units = 1 user.
Are you just hitting your license max?

The last three items solved it for me. I didn't know we really had a max
# of users allowed or how to tell from the license. We were at our
licensed limit.
