HSZ40 error

good morning! (well .. is parts of the world anyways..)

Yesterday afternoon all our users were kicked off the system... I looked at
the daemon log and sure enough:

stopping it anyway

Anyways.. we're running TC1.5, DS20, Unix 4.0E patch 2 i beleive..

I was able to start the service on our backup machine with no problem, I
then stopped it there and restarted it on the initial machine and since then
its fine..

My question is what would cause a 'device access failure'? theres nothing in
any other log
