My question:

> Our machine (AS4100/DU4.0D) have experienced a crash and I would like
> to know what process exactly caused this and why. I know that in
> /var/adm/crash there is all the information you need in order to know

The solution:

The recommendation for this panic, is to obtain and install the latest
patchkit for this version (V4.0D). The patchkits can be found and
downloaded from the following website:
ftp://ftp.service.digital.com/public/Digital_UNIX/

Thanks to all people who responded, but specially to:

   Whitney Latta <latta_at_alf.dec.com>
   Lawrence Philip P <Philip.lawrence_at_astrazeneca.com>

The other people suggested to read manuals about kernel debugging.
