Upgrade Unix from version4.0B to version 4.0D

Dear helpers,

This week I planned an upgrade from DEC OSF/1 UNIX version 4.0B to version 4.0D.
But I have difficulties to begin an upgrade. I don't know how and where to begin.
In our office I have a CD Rom from version 4.0D.
Can you explain what to do with an installation?
