Summary: SCSI errors

Thanks to Thomas Blinn, Seldon Ball Jr and Bryan Lavelle for replying to
my query. I have now tracked the problem down to a bug in the ITPSA driver
which was fixed in Tru64 Unix 4.0F Patchset 4.
